The Community Living Center (CLC) is a VA nursing home. Like the old nursing homes, the Community Living Center resembles “home” as much as possible. There are activities for veterans of all ages. There are family friendly places to visit. Veterans are invited to decorate their rooms. And pets are allowed to visit or live in the community living center. Veterans may stay for a short time or, in rare cases, for the rest of their lives. A nursing home is a place where veterans receive care, including assistance with activities of daily living (such as bathing and dressing) and skilled nursing and medical care. The mission of the Community Living Center is to restore every soldier to their highest level of health. It is also to prevent health decline and provide comfort at the end of life.
Veterans who are outpatients and need more information should contact their primary care providers and PACT social workers. PACT social workers assist with outpatient placement and can refer you to PCLC on your behalf if you inquire about rehabilitation, respite and/or skilled needs. Veterans may sometimes need supportive care and this is determined on a case-by-case basis to ensure Veterans are in the best possible environment to meet their needs.
Hospitalized veterans seeking PCLC are referred directly from hospital social workers involved in the veteran’s care. Each case is reviewed by a panel of experts who assess the veteran’s needs at admission, stay and discharge. The PCLC social worker works with veterans and families to ensure a safe discharge plan is in place that meets the veteran’s needs.
Veterans are considered for admission based on their skill and rehabilitation needs. Once a veteran’s therapeutic, nutritional, pharmaceutical, spiritual, medical and nutritional needs are assessed, an interdisciplinary team works together to promote healing and wellness to ensure the best plan of care while living at CLC. PCLC recognizes the importance of veteran partnerships and hopes to facilitate an atmosphere of care and compassion that ultimately helps our veterans get back on their feet faster than in other traditional rehabilitation settings.
Currently, our facility is limited in its ability to accept walk-ins from the community due to covid. However, we hope to open the portal to community veterans in the future. At this time, we only accept referrals from the Omaha VA for admission.
Visiting hours are limited due to covid and are updated weekly with relatives and families (per national VHA/CLC guidelines).
