Olympia — The Washington State Department of Health has taken disciplinary action or dropped charges against the following health care providers.

The Department’s Health System Quality Assurance Division works with boards, commissions, and advisory committees to develop licensure standards for more than 80 health care professions (eg, dentists, nurses, counselors). Information on disciplinary action against medical doctors and physician assistants can be found at Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions may be sent. media@wmc.wa.gov.

Information about health care providers is available on the agency’s website. Click on “Seek a health care provider’s license“How do I do that?” In the section of Department of Health website (doh.wa.gov) Web site includes information on health care provider licensure status, expiration and renewal dates, disciplinary actions, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe a health care provider has acted unprofessionally are encouraged to call and report their complaint.

Benton County

In July 2022, the Board of Physical Therapy revised the charges against physical therapists. Amy Lynn Lewis (PT60902515) Lewis was charged with unprofessional conduct for failing to comply with a substance abuse control agreement.

Clark County

In June 2018, Hanson was charged in an Oregon district court with mail theft or receiving stolen mail.

Note to editors: Health care providers accused of unprofessional conduct have 20 days to respond in writing to the Department of Health. Then the case goes into the settlement process. If a disciplinary agreement cannot be reached, the case will go to trial.
