Olympia — The Washington State Department of Health has taken disciplinary action or dropped charges against the following health care providers.

The Department’s Health System Quality Assurance Division works with boards, commissions, and advisory committees to develop licensure standards for more than 80 health care professions (eg, dentists, nurses, counselors). Information on disciplinary action against medical doctors and physician assistants can be found at Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions may be sent. media@wmc.wa.gov.

Information about health care providers is available on the agency’s website. Click on “Seek a health care provider’s license“How can I?” In the section of Department of Health website (doh.wa.gov) Web site includes information on health care provider licensure status, expiration and renewal dates, disciplinary actions, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who believe a healthcare provider has acted unprofessionally are encouraged to call and report their complaint.

Clark County

In July 2022, the pharmacy commission rejected the pharmacy assistant application Debra Sue Seitz (VB61161192) She did not agree to the terms of the license. Seitz was charged with burglary, obstructing a law enforcement officer and making false or misleading statements between 2003 and 1997.

Cowlitz County

In August 2022, the Nursing Commission issued the conditions for a licensed practical nurse credential Kenna Courtney LemmonsAlso known as Kenna Courtney Pollock (LP61278856). She must participate in a substance abuse evaluation and monitoring program.

Douglas County

In August 2022, the Pharmacy Commission filed a lawsuit against the pharmacist Jeffrey W. Reynolds (PH00017863) for unprofessional conduct. Reynolds wasted Covid-19 vaccines instead of administering them and fraudulently reported them to two patients.

King County

In June 2022, the Department of Health announced Penny Michelle Phair (MC61202553) A mental health counselor mistakenly licensed her associate. The department revoked the certification and moved the application to “pending” to allow Fire to provide additional information that meets the certification requirements.

West Virginia: In June 2022, the Department of Health announced Virgil Wade McMillion (OP61188818-IMLC) as an osteopathic physician and surgeon in error. The department canceled the certificate and moved it to “pending” status.

Note to editors: Health care providers accused of unprofessional conduct have 20 days to respond in writing to the Department of Health. Then the case goes into the settlement process. If a disciplinary agreement cannot be reached, the case will go to trial.
