Alpine identifies patients who are at risk of a long hospital stay.
Miami And PEORIA, Ill, January 31, 2023 /PRNewswire/ — Alpine health systems It is happy to announce the launch of an AI-based software platform that improves hospital discharge for patients with complex health problems and, in some cases, multiple social needs. The platform enables case managers to quickly identify at-risk patients so that they can be safely transferred to the appropriate treatment facility. Peoria, Illinois– Established OSF Health Care And his creative impulse OSF Creativityalong with Indianapolis-based venture developer High alpha creativityHe has partnered with Alpine to develop a device that could benefit hospitals nationwide.
Hospitals are increasingly filled with complex and difficult-to-treat populations, including those admitted to hospitals with multiple chronic conditions and comorbidities. These patients often have many challenges, including high medical needs, complex family expectations, criminal history, or inadequate home support. Caring for these patients far beyond their geometric mean length of stay (GMLS) in the hospital results in significant costs for providers and missed opportunities to care for patients who need it most.
Starting with a simple rules-based architecture and machine learning (MLL) as well as natural language processing (NLP), Alpine helps critical case managers in the discharge process to raise complex cases earlier, reduce excessive hospital stays and improve patient outcomes. The platform identifies and prioritizes important patient risk factors prior to hospital discharge to ensure an overview of what the best setting and support will be for the patient.
Unlike competitors, Alpine utilizes clinical and non-social aspects of health (SDOH) that may not be part of a patient’s electronic health record or are readily available in the record. Alpine’s software searches historical case notes for additional risk factors, ensuring highly accurate risk profiling in real-time. As Alpine is developed and refined, it will be more capable of identifying factors that put a patient at risk of delay and matching those factors with appropriate recommendations for case managers.
Simplifying the work of hospital case managers should help hospitals attract and retain individuals in these critical areas, so Alpine provides added value to hospitals and health systems.
