Starting with a simple rules-based architecture and machine learning (MLL) as well as natural language processing (NLP), Alpine helps critical case managers in the discharge process to raise complex cases earlier, reduce excessive hospital stays and improve patient outcomes. The platform identifies and prioritizes important patient risk factors prior to hospital discharge to ensure an overview of what the best setting and support will be for the patient.

Unlike competitors, Alpine utilizes clinical and non-social aspects of health (SDOH) that may not be part of a patient’s electronic health record or are readily available in the record. Alpine’s software searches historical case notes for additional risk factors, ensuring highly accurate risk profiling in real-time. As Alpine is developed and refined, it will be more capable of identifying factors that put a patient at risk of delay and matching those factors with appropriate recommendations for case managers.
