The platform identifies and prioritizes important patient risk factors prior to hospital discharge to ensure an overview of what the best setting and support will be for the patient.
Unlike competitors, Alpine utilizes clinical and non-social aspects of health (SDOH) that may not be part of a patient’s electronic health record or are readily available in the record. Alpine’s software searches historical case notes for additional risk factors, ensuring highly accurate risk profiling in real-time.
