job description; The Social Worker II is responsible for patient cases assigned to rotating county residents. Each patient assignment is based on a referral from a medical provider or other agencies for children 0-5 years of age living in Lenoir County. The social worker is responsible for: 1. Assisting in accessing community resources to ensure positive health outcomes 2. Educating clients and families on the appropriate use of medical services 3. Monitoring any risk factors that may affect positive health outcomes. Using Life Skills Progression, SWYC and Community Needs Assessment; develop and implement a plan of care to ensure healthy outcomes; and information data to the North Carolina Department of Medical Assistance database – Virtual Health (VH) Social Worker II is responsible for the quality of health care provided to patients served by the agency. This work requires specialized knowledge of advanced social work assessment using concepts and practices in the delivery of patient care.
1. Principles and methods used in high level social work

Minimum Training Required: The social worker (MSW, BSW, or SW II qualifications) must have at least one year of experience in health and human services and experience working with pregnant women and their families.
